St. Liz Hospice believes that when treating a patient with a life-limiting illness, there is no place like home. Wherever home is for you, be it your private residence, a Board and Care Facility, Assisted Living Facility, or Skilled Nursing Facility, we will be there for the patient. St. Liz Hospice staff are seasoned professionals who can provide comfort, support, and care when you need it most. Our goal is to make sure that for the remaining days of the patient, they should have the dignity and quality of life that they deserve. Our hospice team will make sure that our patients will have peace of mind and confidence knowing that their remaining days will be moments of building memories together with their loved ones.
Depending on the patient’s needs, our physicians and nurse practitioners can also make house calls, who are working closely with your hospice team of nurses, social workers, home health aides, chaplains, spiritual care coordinators, and volunteers to make sure that your needs are met. Once admitted to our hospice program, a Plan of Care is developed, identifying the intervention needed by the patient. The goal of the hospice Plan of Care is to provide the best possible level of care in the most appropriate setting to the patient’s current clinical condition.
