For hospice care at the patient’s private residence, Board, and Care or Assisted Living facilities, there is a level of care called Continuous Care (CC). This is a level of care where we send a skilled nurse to the patient’s home to do bedside nursing. This level of care is usually triggered by a call from a caregiver regarding the patient’s current condition. Clinical conditions like agitation, anxiety, uncontrolled pain, etc., are conditions that meet Medicare criteria for CC.
