St. Liz Hospice is a Medicare and MediCal (Medicaid) Certified Hospice Agency that obtained its Medicare Provider Number on
June 14, 2001. It is also a Joint Commission Accredited hospice agency and is a member of National Hospice and Palliative Care Organization (NHPCO) and California Association for Health Services at Home (CAHSAH).
With more than
20 years of service, St. Liz Hospice managed to earn the trust and confidence of the communities it served. Through hard work and dedication, St. Liz Hospice was able to be contracted with various prominent health plans and institutional health organizations like Kaiser Permanente, Optum Care (formerly Health Care Partners), Regal Medical Group, Lakeside Medical Group, Cal-Optima, Los Angeles County Department of Health, and the California Department of State Hospitals.
Seeing the future needs and the evolution of the hospice industry, in December of
2018, a new Mission and Vision Statement was adapted by St. Liz Hospice, together with the Core Values and Philosophies that St. Liz Hospice wholly embrace as the foundation of its future corporate growth and development.
